CLC's Medicare Advantage Plan

In 2020, CLC became a part owner of Perennial Advantage, a Medicare Advantage plan offered to residents in our three Denver area Life Plan/Continuing Care communities. Having our own Medicare Advantage plan, along with in-community health clinics, offers a higher level of coordinated care to those who have chosen a CLC community as their home.

As an operator-owned Medicare Advantage (MA) network, Perennial Advantage’s mission is to provide better outcomes, increased satisfaction, and lower health costs to CLC residents. It all starts by bringing attention to the preventive lifestyle and chronic care management within our communities. 

CLC is one of a few senior living providers partnering to provide Perennial Advantage, which is available to residents of Clermont Park, Holly Creek, and Someren Glen.  


Perennial Advantage Plans

Unlike traditional Medicare Advantage plans, Perennial offers continual enrollment due to the special nature of each plan. 

Freedom (HMO)  

Beginning in 2023, the Freedom Plan is available to any resident living in one of CLC’s Arapahoe County communities and who live in Residential/Independent Living neighborhoods. We hope to be able to offer this plan in Denver County in 2024.  

Available to residents of Holly Creek and Someren Glen 

Concierge (HMO C-SNP) 

The Concierge plan is available to residents who have specific chronic health conditions like diabetes or heart disease.  

Available to residents of Clermont Park, Holly Creek, and Someren Glen 

Strive (HMO I-SNP) 

The Strive plan is just for those Medicare beneficiaries who meet the long-term care requirements.  

Available to residents of Clermont Park, Holly Creek, and Someren Glen
